Medicare CDM

Referring Under a Chronic Disease Management Plan

For conditions lasting six months or more. Terms and conditions apply.

How the CDM pathway works

If a condition has lasted, or is predicted to last, at least six months, the patient may be eligible for Medicare assistance under the Chronic Disease Management scheme: up to five Medicare-rebatable exercise physiology sessions per calendar year.

The GP completes the CDM plan and the allied health referral form, and we schedule the baseline assessment from there. Questions about eligibility or the paperwork? Call us and we will walk it through with you or your practice staff.
